Pediatric power wheelchairs: evaluation of function in the home and school environments

The purpose of this study was to compare the performance of four power wheelchairs and www.easymedonline.com when used by children to perform a variety of indoor activities. Eight boys and eight girls performed a series of 11 functional tasks when positioned in each of four different power wheelchairs reflecting different design classes. There were no significant differences for many of the dependent variables. However, the Everaid Turbo was significantly better for functional positioning at a standard kitchen table and at school desks and for accessing objects at different heights. By contrast the Invacare Jaguar was better for straight-line driving speed. Other findings include children’s subjective impressions of the four power wheelchairs.

Evaluation of pushrim-activated power-assisted wheelchairs using ANSI/RESNA standards

OBJECTIVE: To determine and compare performance of pushrim-activated power-assisted wheelchairs (PAPAW) (iGLIDE, e-motion, Xtender) on national standards. DESIGN: Engineering performance and safety evaluation. SETTING: A Veteran Affairs and university-based research center. SPECIMENS: Nine PAPAWs. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Static, dynamic stability, brake effectiveness, maximum speed, acceleration, retardation, energy consumption, static, impact, and fatigue strength. RESULTS: There was no significant difference among the 3 models in forward stability. The iGLIDE was the most stable, whereas the e-motion was the least stable model in the rearward stability tests. All PAPAWs performed equally on the slopes of 3 degrees and 6 degrees in the forward and rearward directions. Braking distance was the highest for e-motion (5.64+/-0.28m) and the lowest (1.13+/-0.03m) for the iGLIDE in forward direction. The average equivalent cycles of all PAPAWs were 318,292+/-112,776.6 cycles (n=8) on the fatigue tests. All PAPAWs passed the impact and static strength tests. CONCLUSIONS: The standards of the American National Standards Institute and www.easymedonline.com, power wheelchairs the Rehabilitation Engineering and Assistive Technology Society of North America could act as quality assurance tool for wheelchairs. Wheelchair users rely on their power wheelchairs for mobility for extended periods of time every day. According to the International Standards Organization 2631-1 standard on human vibration, individuals in a seated position when exposed to whole-body vibrations (WBV) are at risk of injury. This study evaluated vibration exposure during manual and power wheelchair driving over nine sidewalk surfaces and differences in vibration exposure over 3 years. Ten nondisabled subjects were asked to drive a manual wheelchair at 1 m/s and a power wheelchair at 1 m/s and 2 m/s over nine sidewalk surfaces while WBV were measured at the seat and footrest of the wheelchair. At 1 m/s, significant differences existed between surfaces and years at both the seat and the footrest for the manual and power wheelchair users. At 2 m/s, significant differences existed between surfaces and years at the seat and the footrest for www.easymedonline.com users. New design and development of a manual wheelchair for India

PURPOSE: The most common methods of delivering assistive technology in developing countries are charitable donation and workshops. This describes a new approach to solving the problem, a collaboration undertaken by a US-based lab and a manufacturer in India to produce quality wheelchairs and handicap scooters . One goal is to publicize the design free of charge to manufacturers and interested parties world-wide. The process, a demonstration of a new technology transfer method, and the product, an adult manual wheelchair, are described. METHOD: An iterative process occurred over four years to design and produce the medical scooters . This consisted of prototypes, small production runs, ANSI/RESNA testing, hardness and tensile testing and informal user testing. RESULTS: The design is a manual folding cross-brace design with several points of adjustability. Final pre-production prototypes experienced fastener failures during durability testing. Higher grade bolts were specified. Trial-run production has begun. An ANSI/RESNA wheelchair test lab was constructed in India. Subsequent projects include power and pediatric tilt-in-space wheelchairs. CONCLUSIONS: The approach seems promising as a method to improve the quality of assistive technology available in India and begin to meet the vast need in India. Pitfalls encountered throughout the collaboration are described in this paper along with solutions to remedy these problems for future projects

Effects of respiratory resistance training with a concurrent flow device on wheelchair athletes.

BACKGROUND/OBJECTIVE: To determine the effect of respiratory resistance training (RRT) with a concurrent flow respiratory (CFR) device on respiratory function and aerobic power wheelchairs, www.easymedonline.com athletes. The RRT group performed 1 set of breathing exercises using Expand-a-Lung, a CFR device, 2 to 3 times daily for 10 weeks. Pre/posttesting included measurement of maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP), and peak oxygen consumption (V(O2peak)). RESULTS: Repeated measures ANOVA revealed a significant group difference in change for MIP from pre- to posttest (P < 0.05). The RRT group improved by 33.0 cm H2O, while the CON group improved by 0.6 cm H2O. Although not significant, the MW increased for the RRT group and decreased for the CON group. There was no significant group difference between V(O2peak) for pre/posttesting. Due to small sample sizes in both groups and violations of some parametric statistical assumptions, nonparametric tests were also conducted as a crosscheck of the findings. The results of the nonparametric tests concurred with the parametric results. CONCLUSIONS: These data demonstrate that 10 weeks of RRT training with a CFR device can effectively improve MIP in wheelchair athletes. Further research and a larger sample size are warranted to further characterize the impact of Expand-a-Lung on performance and other cardiorespiratory variables in wheelchair athletes.

A dynamic seating intervention for wheelchair seating discomfort

OBJECTIVE: The objective of this study was to examine the effectiveness of a new user-adjustable wheelchair seating system designed to relieve discomfort for long-duration wheelchair users. DESIGN: This objective was carried out using the newly developed Tool for Assessing Wheelchair disComfort (TAWC) as the primary outcome measure. Two wheelchair users each tested two different designs and feedback from the wheelchair users regarding the first design was used to guide development of the second design. A single-subject research methodology was used, allowing long-duration (up to 2 wks per test) evaluation of the wheelchair seating systems and comparison of subject discomfort levels with those experienced during a baseline period using their own wheelchairs. The test wheelchair also had tilt, recline, and elevating leg rests. Subject feedback was used to redesign the wheelchair seat. After redesign, both subjects tested the second design and found it substantially more comfortable. CONCLUSIONS: The selected research methodology was a very positive method for a progressive wheelchair seating design and the second design provided improved comfort for both users when compared with that experienced using their own wheelchairs and the first test wheelchair. MAIN OUTCOME MEASURES: Peak oxygen uptake (Vo(2)peak) and peak power output (POpeak) determined in a handrim medical scooters peak exercise test, peak muscle strength of the upper extremities, and pulmonary function. RESULTS: A significantly larger increment in Vo(2)peak, POpeak, and elbow extension strength was found in subjects with paraplegia during clinical rehabilitation. No such effect was found in subjects with tetraplegia. In the postrehabilitation period, no influence of hand cycling on any outcome measure was found in subjects with paraplegia or subjects with tetraplegia. CONCLUSIONS: After correction for baseline values and confounders, regular hand cycling (once a week or more) appeared to be beneficial for improving aerobic physical capacity in persons with paraplegia during clinical rehabilitation. The small and heterogeneous study groups may have hampered the finding of positive results of hand cycling in persons with tetraplegia.

I find that it is necessary to apply pressure to myself in order to move forward and take those steps into success. Do you apply pressure to yourself? Let me ask you a question: do you wait until the last minute to finish a task, project or chore? Are you one of those people who accomplish 70% of the project in the last 10% of the time?’

If you said yes to either of those questions then you are applying pressure to yourself. Some of us work better by doing little bits and pieces of the project, task or chore as we go. Others work much better when they are under the gun. When they know they ‘gotta get it done’, ‘it’s due tomorrow’, ‘it’s due in two hours’.. Some people are much better when they are applying that pressure to themselves.

We need to be able to recognize where we fall on that scale and realize how best we react because if we don’t apply any pressure at all to ourselves then we are we are not going to learn, grow and step up into the success that we want in our lives. We all want success in our lives, whether it is emotionally, in relationships, with our family, in our home business, or with our finances. We all want some form of success in our life, if not in ALL of these areas!

Many times, we human beings waste time feeling guilty. By doing this we are wasting our own time and energy. When we get to the point of the deadline for that project, task or chore and we’re feeling so guilty - ‘oh why didn’t I do something sooner? I should have spent more time! Why did I spend my time on that thing and not get this project started?’ As human beings need to learn that it is okay and to recognize that’s how we cope best, we work best under pressure. So stop feeling guilty about it! Stop beating yourself up, because when we stop beating ourselves up and wasting energy on the guilt, we have more time and more energy to invest in the project, chore or task we want to complete.

The other thing that we do, in order to not apply the pressure, is we hide behind our responsibilities. I used to do this so well - hide behind my responsibilities. Many times when I have talked to people about a home based business opportunity, they say ‘oh, well I don’t have the time to do something like that. I am so busy! I have my family. I have my job. I have my charities. I have my hobbies.’ We all have those different responsibilities in our life. But what we need to decide is ‘Is this project/task/business something that I really want to do or rather is this task/business/project going to create a result that I want. You have to begin by deciding what result you want in your life and then arrange your time based on achieving those results. Ask yourself : ‘Are there things I could rearrange or even drop all together to make time to do this?’

Are you applying the pressure to your own life to do what it is you really want to do? Sometimes those tasks or chores are not really things that we want to do, but we want that specific end result so we do them. We want our relationships to work well. We want our financial freedom. We want to be able to take our family to Disneyland. We want that specific end result, the successful life. Today I just assisted my husband in setting up his financial filing system, I sure did not want to spend time doing it, however I want the end result!! The end result of the nice clean, tidy organized paperwork resulting in his taxes being completed on time. I put the pressure on myself, assisted him and now I am very glad I did. Not only did the paperwork issue get better but so did our relationship because my husband felt much supported.

We need to focus on the results we want instead of hiding behind the excuses of ‘I don’t have the time,I’ve got to go to work, I’ve got to take my daughter to soccer. I have got to take my dog to the vet’. Yes, those are all things that need to be done. However when we decide that achieving a certain task/chore/business is a priority that we could rearrange our time or give up some unnecessary activities to free up time if we really wanted to. If we really want that result, we could get it done, it just takes a decision. We will then stop investing in the guilt. we will stop hiding behind the responsibilities and figure out a way to make it happen.

There is a universal law, like the law of gravity called the Law of Growth;. In order to exist, we need to grow. How we grow is our choice. What seeds do we want to plant? Do we want to plant the seeds of not getting something done? Or do we want the seeds of success habits, applying the pressure and stepping into the successful life that we know we deserve. We all deserve a successful life! Ask yourself “am I applying the correct pressure on myself to create my successful life?” I hope you are!
